Job summary

Alternate Solutions Health Network is seeking an Intake Admission Specialist to support remote home health and hospice referrals, coordinating with physicians, staff, and community resources to transition patients to services.

The role requires an Associate degree or equivalent, two years healthcare experience (one in home care), and strong communication; you'll input patient data, ensure CMS compliance, and partner with referral sources.

Qualifications

• Associates degree or equivalent healthcare/home care experience required.
• Two years of healthcare experience with one year in home care preferred.
• Experience in Medical Terminology is preferred.
• Knowledge of government regulations and private payer practices.

Responsibilities

• Process all daily patient referral and intake operations.
• Coordinate care with multiple service providers and call physicians to obtain orders.
• Follow up with Start of Care (SOC) and services to measure patient satisfaction.
• Report delays, non-admits, and SOC issues.
• Follow agency policies and procedures and maintain relationships with referral sources.
• Support Community Educators and Community Liaisons in referrals and client services.
• Ensure CMS guidelines compliance and state/federal regulatory requirements.
