Job summary

Saint Luke's Home Care and Hospice is seeking a full-time RN Clinical Intake Coordinator to lead clinical screening, evaluation, and admission of patients to Home Health and Hospice services, ensuring Medicare, Medicaid, and commercial insurance requirements are met.

This role requires 2 years of RN experience (KS/MO), strong communication, critical thinking, and independence in a fast-paced environment; weekend coverage, and Epic documentation are part of the duties.

Qualifications

• 2 years of applicable RN experience required.
• KS and MO RN licenses and NURSYS verification required.
• Associate degree in nursing completed and active.

Responsibilities

• Review and evaluate Home Health and Hospice referrals for eligibility.
• Coordinate referral processing from hospitals, clinics and other providers.
• Identify missing clinical information and obtain necessary documentation.
• Interpret Medicare, Medicaid, and insurance requirements for admission.
• Collaborate with physicians and clinical teams to facilitate timely admissions.
• Document referral activity accurately in Epic and maintain records.
