Qualifications

• Active certifications in fitness, wellness, or related clinical areas preferred.

Responsibilities

• Provide guidance and support to clinical members, helping them understand benefits and resources.
• Conduct health assessments and identify at-risk individuals for targeted coaching.
• Track member progress and document sessions in EMR.
• Collaborate with care team to create individualized care plans.
• Educate, outreach, advocate, and assist with navigating the healthcare system.
• Follow protocols for chronic disease management using clinical judgment.
• Use data and systems to help members access cost-effective care.
