Job summary

Altea Healthcare is seeking a licensed Advanced Practice Provider to join our Chronic Care Management program in a part-time, fully remote capacity. This role focuses on non-face-to-face CCM services for an established patient population, ensuring continuity of care, care coordination, and proactive patient engagement in CMS CCM guidelines.

Candidates must hold an active Missouri State license and be comfortable working independently in a remote environment while meeting daily productivity

Qualifications

• Active Missouri NP license.
• National board certification as NP.
• Experience in chronic disease management preferred.
• Comfort with remote work and telephonic engagement.
• Strong documentation and organizational skills.

Responsibilities

• Provide CCM services to patients with two or more chronic conditions.
• Complete a minimum of 20-2 CCM encounters per workday.
• Non-face-to-face patient interactions via phone and follow-up coordination.
• Develop, update, and manage electronic care plans.
• Coordinate care with PCPs, specialists, facilities, and ancillary services.
• Document all patient interactions in the EMR.
• Ensure CCM activities meet CMS, payer, and organizational standards.
• Identify care gaps and escalate clinical concerns appropriately.
• Participate in quality improvement initiatives for chronic disease management.
• Maintain productivity, quality, and documentation benchmarks.
