Job summary

UnitedHealth Group is recruiting a Care Coordinator to manage a panel of members with chronic and complex health care needs in Idaho. The role supports clinical and non-clinical activities to promote person-centered care coordination, with remote work flexibility for certain areas.

The position requires Idaho licensure (RN or social work), 1+ year Medicaid/Medicare or SDoH experience, and the ability to travel up to 50% while maintaining PHI security and high-speed internet access.

Qualifications

• Idaho license in nursing or social work-related field.
• 1+ years Medicaid/Medicare or SDoH experience.
• 1+ years MS Office experience (Word/Excel/Outlook).
• Driver's license and travel up to 50% of time as needed.
• Home workspace with ability to protect PHI and secure internet.
• Reside in a location with access to high-speed internet per policy.

Responsibilities

• Serve as primary care manager for dual eligible members.
• Conduct comprehensive needs and wellness assessments with client-centered plans.
• Collaborate with care team and community resources to implement plans and remove obstacles.
• Assist with obtaining HCBS supports and services.
• Provide referrals and linkage to internal or community resources.
• Support discharge planning and care transition after ER, hospital, or SNF.
• Educate and coach for self-management and healthy lifestyle changes.
• Advocate for member needs within the health care team.
• Handle nonclinical questions with providers and facilities.

Education

Idaho license (RN) or behavioral/mental health license (LCSW/LMSW/LSW/LCPC/LPC/LMFT/LAMFT)
