Job summary

UnitedHealth Group is seeking a Care Coordinator to manage a panel of members with chronic and complex health needs in Idaho, with remote work flexibility. This role focuses on person-centered care, linkage to services, and collaboration with a broad care team.

The ideal candidate holds an Idaho license (RN or related), has Medicaid/Medicare or SDoH experience, and is proficient with MS Office. Travel up to 50% may be required depending on member needs.

Qualifications

• Active Idaho license in RN or related profession (LCSW, LMSW, etc.).
• 1+ year experience with Medicaid/Medicare or SDoH.
• Proficient with MS Office (Word, Excel, Outlook).
• Valid driver's license and reliable transportation.
• Ability to travel up to 50% of time to meet members/providers.
• Access to a dedicated home workspace and PHI security.
• Reside in Idaho with access to high-speed internet.

Responsibilities

• Serve as the primary care manager for dual eligible members.
• Engage members face-to-face and/or telephonically to complete comprehensive needs or wellness assessments.
• Partner with internal care team and community resources to implement care plans.
• Assist members with obtaining HCBS supports and services.
• Provide referrals to internal or community providers as appropriate.
• Support proactive discharge planning and care transitions after ER/inpatient/SNF.
• Educate and coach members for self-management of health needs.
• Advocate for members to ensure their needs are represented by the care team.
• Support nonclinical questions from providers/ facilities and connect to relevant resources.
