Job summary

Israelvcforum is seeking a Remote Chronic Care Coordinator to assist in transforming healthcare delivery. This role involves telephonic encounters with patients and developing detailed care plans within care plan templates.

As a part-time position, the coordinator will set their own hours and carry a patient panel of at least 100 patients per month. There is potential for this role to become a full-time W2 position, including benefits.

Qualifications

• A minimum of two years of clinical experience in a related field.
• Current active nursing license in Michigan.
• Hands-on experience with EMR and Windows applications.

Responsibilities

• Carry a minimum of 100 patients monthly.
• Develop care plans for patients.
• Conduct telephonic encounters with patients.
