Esrun Health, a division of Harris, is seeking a Remote Care Coordinator to provide telephonic care to patients with chronic conditions on a part-time, contractor basis. You will set your own schedule from home and manage a patient panel while complying with HIPAA requirements.

The role is productivity-based with pay per completed patient encounter and escalating tiers. Training is self-led, with growth opportunities into a full-time position and benefits if available.

Qualifications

• Certified Medical Assistants with medical training and licensure.
• Two years of clinical experience, preferably in pain management.
• Hands-on experience with Electronic Medical Records and HIPAA-compliant home work setups.
• Ability to take initiative, show good judgment, stay organized and manage time effectively.
• Proficient with computer programs; Windows and Office 365 usage.
• High-speed internet and a functioning computer; no Chromebooks or tablets.

Responsabilités

• Abide by the plan of care and orders of the practice.
• Provide prevention and intervention for multiple disease conditions via coaching.
• Develop positive patient interactions for practice partners.
• Increase revenue by creating billable Care Management episodes.
• Attend mandatory meetings and document care activities.
