Job summary

Vitasora Health is seeking a remote Care Coordinator to manage patients in CCM and RPM programs. You will blend clinical expertise with patient engagement skills, using motivational interviewing and health coaching to drive behavior change and improve outcomes for chronic conditions.

You will onboard patients, coordinate with care teams, and ensure HIPAA-compliant documentation while addressing social determinants of health and barriers to access, especially in underserved communities.

Qualifications

• All certifications and licenses must be current and in good standing and will be verified through primary source before hire.
• Active, unencumbered RN or LPN/LVN compact license and related certifications.
• Minimum 1 year of clinical experience in chronic care management, telehealth or related setting; 3+ years preferred.
• Experience with motivational interviewing and health coaching in a clinical or patient engagement setting.
• Proficiency in EHR systems, telehealth platforms, and remote patient monitoring tools.

Responsibilities

• Engage patients via monthly outreach using motivational interviewing and health coaching techniques.
• Develop and update CCM care plans based on assessments and patient priorities.
• Coordinate care with physicians, nurses, social workers, and specialists.
• Maintain HIPAA-compliant documentation in the EHR and track engagement metrics.
• Identify barriers to care and advocate for patient-centered solutions.
