Job summary

Vitability Health seeks a compassionate Chronic Care Manager (RN) to support patients with multiple chronic conditions remotely. You will develop personalized care plans, conduct assessments, coordinate with care navigators, and promote adherence through telehealth and remote monitoring, while complying with HIPAA and CMS guidelines.

Qualifications

• Active RN license in New Jersey or a valid Compact RN license.
• Experience in Chronic Care Management (CCM).
• Acute care nursing experience is required.
• Patient-centered approach and strong communication abilities.
• Ability to lead and collaborate with care coordinators in a team.
• Comfort using EHR systems, telehealth platforms, and remote monitoring tools.

Responsibilities

• Provide ongoing chronic care management to a caseload of patients with multiple chronic conditions.
• Develop and maintain personalized care plans reflecting patient goals, challenges, and needs.
• Conduct comprehensive assessments of medical, social, and lifestyle factors influencing health.
• Monitor progress, identify changes early, and adjust plans for better outcomes.
• Educate patients and families on disease management and healthy lifestyle choices.
• Use telehealth and remote monitoring to stay connected between visits.
• Collaborate with care navigators (social workers) to ensure high-quality care.
• Coordinate appointments, tests, and follow-ups to reduce barriers to care.
