The position operates in a hybrid schedule (remote and in-office) with a 40-hour work week, requiring residency near the Denver metro area for in-home visits.

Qualifications

• Bachelor’s degree in a related field.
• One year of work-related experience in comprehensive case management.
• Knowledge of laws relating to children and families (preferred).

Responsibilities

• Determine client eligibility by gathering collateral information.
• Complete a face-to-face assessment of clients at home or in community settings.
• Attend court appeal hearings and prepare relevant documents.
• Maintain client care plans and reassess for cost-efficient services.
• Negotiate rates and plans with Medicaid and other providers.
