Job summary

Jefferson County Human Services in Colorado is seeking a Long-Term Care Case Manager to provide client-facing case management services for elderly and disabled adults and children with chronic or terminal illnesses. The role includes determining functional eligibility for Medicaid service plans, negotiating rates with providers, and following up on care options; and assisting with denied applications.

Work is in a hybrid environment with 4x10 hour weeks or 5x8, depending on team needs.

Qualifications

• Bachelor’s degree in a related field.
• One year of work-related experience.

Responsibilities

• Determine client eligibility by gathering information from hospitals, nursing homes, and home health services.
• Complete face-to-face assessments of clients.
• Attend court appeal hearings to defend denied applications.
• Prepare all legal documents for appeal hearings.
• Negotiate rates and plans with Medicaid providers and other providers.
• Maintain care plans by reassessing for cost-efficient service plans.
• Adhere to state cost containment regulations.
• Ensure family members are informed of options.
