Job summary

CareCollab is expanding its Children’s Health Home program to deliver comprehensive, person-centered care coordination for children and youth in home and community settings.

The Children’s Care Manager will assess needs, develop care plans, and coordinate services across medical, behavioral health, education, and social supports. The role blends remote work with field visits, requiring travel within the service area.

Qualifications

• Bachelor’s degree in Social Work, Human Services, Psychology, Nursing, Public Health, Education, or related field; or RN with 2+ years experience; or Master’s with 1 year of relevant experience.
• Experience with children, adolescents and families in healthcare, behavioral health, child welfare, social services, or care/case management.
• Proficiency with EHRs, care management platforms, Microsoft Office and other technology tools; ability to collaborate with families, providers, schools and teams.

Responsibilities

• Coordinate care and services for children and adolescents across multiple systems.
• Develop Plans of Care in collaboration with families and multidisciplinary teams.
• Monitor progress, adjust care plans, and facilitate transitions of care.
• Maintain contact with members through home visits, telehealth, and community visits.
• Ensure documentation, compliance with Health Home standards and Medicaid requirements.
