Job summary

Medacs Healthcare is seeking an experienced CHC Nurse Assessor (Band 6) for a fully remote locum role. The post involves conducting CHC assessments, managing a caseload of complex health needs, and coordinating care packages within NHS services and local authorities.

The ideal candidate holds current NMC registration (RGN/RMN/RNLD) with CHC or related community health experience, strong decision-making skills, and excellent communication with clinicians, service users and families.

Qualifications

• Current NMC registration as an RGN, RMN or RNLD.
• Right to Work in the UK.
• Experience in CHC, community nursing, complex care, mental health, learning disabilities or clinical case management.

Responsibilities

• Complete CHC assessments and reviews.
• Manage a caseload of individuals with complex health needs.
• Review and coordinate packages of care.
• Support NHS-funded Nursing Care and Fast Track pathways.
• Maintain accurate clinical records and documentation.
