Job summary

SCM Associates, Inc. is seeking a Catastrophic Nurse Case Manager to identify, assess, plan, coordinate, implement, monitor, and evaluate care for individuals requiring case management services.

The role primarily works from a home office with several patient appointments in person each week, and travel to appointments is a requirement. You will collaborate with clients, families, physicians, and other health care team members to ensure quality, cost-effective outcomes.

Qualifications

• BSN preferred.
• CCM preferred.
• Three years of full-time related Catastrophic Case Management experience.

Responsibilities

• Identify and assess client needs to plan care.
• Coordinate services and resources to implement the plan.
• Monitor services for quality and cost-effectiveness.
• Provide ongoing instruction to client and family regarding the illness and care options.
• Travel to patient appointments regularly.
• Communicate with family, physician, and care team to update progress.
• Document outcomes and progress in client records.
