Job summary

A health care service provider is seeking a Service Coordinator to facilitate supportive services for elderly and disabled participants. This role is primarily remote, with occasional travel required to visit participants in their home. The ideal candidate will have a bachelor’s degree in social work, psychology, or related field, or relevant experience, and must possess reliable transportation. Strong organizational and communication skills are essential.

Qualifications

• Bachelor’s degree in a relevant field or equivalent experience.
• Reliable transportation and valid driver's license required.
• Bilingual preferred.

Responsibilities

• Coordinate and facilitate supportive services for elderly and/or disabled participants.
• Manage an active caseload of 70-80 participants.
• Complete person-centered assessments to assist members in obtaining services.
• Notify members of their right to choose service providers.
• Report incidents and allegations of neglect/abuse promptly.

Education

Bachelor’s degree in social work, psychology, or related field

At least 3+ years of experience in a social service or health care related setting
