Limitlessli is seeking a remote Case Manager to support a US-based healthcare client. You will guide patients through their care journeys, coordinate services, and advocate for high-quality outcomes while maintaining clear communication with families and providers.

Ideal candidates hold a BS in Nursing with a valid USRN NY license, at least 2 years of clinical experience, and strong English communication skills. The role operates EST hours, Monday–Friday, from home.

Competenze

• Valid USRN - NY nursing license is required.
• Strong written and verbal English communication.
• Experience working with patients who have difficulty expressing needs.

Mansioni

• Assist patients in evaluating clinical status, medications, and treatment options.
• Conduct concurrent and retrospective reviews to assess medical necessity and level of care.
• Interview clients to assess situational needs and determine required services.
• Document case histories, prepare reports, and liaise with families and services.
