Job summary

CareStar, Inc. in Ohio is seeking a Case Manager to assess client needs, develop personalized care plans, and coordinate services across Medicaid and community supports in Trumbull, Ashtabula, Portage, and Mahoning Counties.

The role offers remote work with field visits, a full-time schedule, and opportunities to impact lives in the community through compliant, person-centered care coordination.

Qualifications

• Bachelor's degree in Social Work, Counseling, Psychology, Nursing, or a related field.
• Licensed Social Worker (LSW), Licensed Independent Social Worker (LISW), Licensed Practical Nurse (LPN) or Registered Nurse (RN).
• Minimum 1 year of experience in home and community-based services (within the last 5 years).
• Experience with individuals with chronic conditions or severe and persistent mental illness.

Responsibilities

• Conduct face-to-face eligibility evaluations and assessments.
• Be available during business hours 8:00 a.m. to 5:00 p.m. to complete duties.
• Travel to clients' homes within a reasonable distance.
• Develop and maintain individualized, person-centered care plans and health and safety actions.
• Coordinate and authorize services across behavioral health, medical, and community systems.
