Job summary

Community Health Plan of Washington in Seattle is seeking a Care Coordination professional to support members under RN or LICSW supervision. You will assist with assessment, planning, facilitation and advocacy to improve health outcomes.

Qualifications include a bachelor’s in social work or related field, at least one year in care coordination or care management, and experience with Microsoft Office and EMR systems. Vaccination and background checks apply.

Qualifications

• Bachelor’s degree in social work, nursing or related health field; 1 year in care coordination or care management preferred
• Experience as medical assistant or behavioral health technician or related roles preferred; LPN considered
• Minimum of 1 year clinical and/or care management experience required
• Proficiency with Microsoft Office products
• Knowledge of Medicare and Medicaid regulations
• Experience in care management in a health plan or community agency preferred

Responsibilities

• Coordinate and communicate with providers to arrange follow-up appointments and obtain records
• Facilitate services and coordinate care across physical and behavioral health providers
• Document the care management process in the medical record system
• Develop and monitor plans of care with patient-centered interventions
• Support crisis and non-crisis member needs as part of the care team
• Participate in data collection and outcome monitoring
