Placements24 is seeking a highly organized Remote Case Manager to support clients in a broad service area. You will assess needs, create personalized care plans, and connect clients with essential resources from a remote location.

Strong communication and problem-solving skills are essential to coordinate care and maintain thorough records. Ideal candidates hold a bachelor’s degree in Social Work, Psychology, or a related field and have at least two years of case management experience.

Qualifications

• Bachelor's degree in Social Work, Psychology, or related human services field.
• Minimum 2 years of experience in case management or similar role.
• Proficiency with case management software and virtual communication tools.
• Strong understanding of community resources and social service delivery systems.
• Excellent organizational, time management, and problem-solving abilities.
• Ability to work autonomously from a remote setting.

Responsibilities

• Conduct needs assessments for clients via virtual meetings and calls.
• Develop, implement, and monitor individualized case management plans.
• Coordinate services with internal and external providers (healthcare, housing, employment).
• Provide advocacy and support to clients navigating complex systems.
• Maintain accurate client records and progress notes in the digital system.
• Review case plans and progress regularly to ensure optimal outcomes.
