Job summary

TimelyCare is seeking a Case Manager to provide telephonic and virtual case management, crisis support, and care coordination across a nationwide remote footprint. You will review safety concerns, coordinate with providers and community resources, and develop care plans tailored to member needs, including higher levels of care when appropriate.

Qualifications

• Master’s degree in mental health is required.
• Active and unrestricted clinical license preferred or required by role scope.
• 3–5 years of clinical experience, especially in crisis/case management.
• Experience with telehealth, care coordination, and crisis intervention.
• Strong documentation, communication, and customer service skills.

Responsibilities

• Provide telephonic and virtual case management, care coordination, crisis support, and member navigation.
• Take CrisisNow calls during assigned coverage times and provide immediate support to callers.
• Remain available in queue while awaiting CrisisNow calls or other call center responsibilities.
• Assess risk and escalate to emergency services when appropriate.
• Coordinate care with TimelyCare providers, school resources, external partners, and community resources.
• Develop and monitor individualized care plans and advocate for appropriate levels of care.
• Maintain accurate documentation and use multiple communication channels as needed.
