Job summary

Piper Companies seeks a Case Manager to support a health insurance and managed care organization. This fully remote role is open to candidates located in North Carolina and involves collaborating with interdisciplinary healthcare teams to improve member outcomes and coordinate services for cost-effective care.

Requirements include an active RN, RD, or LCSW license with at least three years in clinical or care coordination roles; Master’s in Social Work may supplement for senior positions.

Qualifications

• Active RN, RD, or LCSW license with at least three years of clinical, care coordination, or case management experience.
• Master's in Social Work may be considered for select positions with five years of related experience.
• Behavioral Health-focused candidates may qualify with alternative applicable licensure and at least three years of direct clinical experience.
• Must possess and maintain a valid North Carolina or compact multi-state clinical license in good standing.
• Strong assessment, communication, care planning, documentation, and interdisciplinary collaboration skills.

Responsibilities

• Conduct comprehensive evaluations to identify member needs, risks, goals, and support requirements, creating individualized care strategies.
• Partner with members, families, healthcare providers, and community organizations to coordinate services across the healthcare continuum.
• Monitor member progress through regular outreach, reassessments, and care plan updates to ensure optimal clinical and resource utilization outcomes.
• Provide education, advocacy, and guidance to encourage preventive care, treatment adherence, and informed healthcare decisions.
• Maintain accurate documentation of assessments, interventions, care coordination activities, and member outcomes while ensuring regulatory and organizational compliance.
