Job summary

Piper Companies is seeking a Case Manager to support a leading organization in health insurance and managed care. This fully remote role is available to candidates located in North Carolina and involves collaboration with interdisciplinary healthcare teams to improve member outcomes, coordinate services, and promote quality, cost-effective care.

Qualifications

• Active RN, RD, or LCSW license required with minimum 3 years in clinical/care coordination/case management.
• Master’s in Social Work may be considered for select positions with 5 years related experience.
• Behavioral Health-focused candidates may qualify with alternative licensure and 3 years direct clinical experience.
• Must possess and maintain a valid NC or compact multi-state clinical license in good standing.

Responsibilities

• Conduct comprehensive evaluations to identify member needs, risks, goals, and support requirements; create individualized care strategies.
• Partner with members, families, providers, and community organizations to coordinate services across the healthcare continuum.
• Monitor member progress with outreach, reassessments, and care plan updates to optimize outcomes.
• Provide education, advocacy, and guidance to encourage preventive care, adherence, and informed healthcare decisions.
• Maintain accurate documentation of assessments, interventions, and outcomes, ensuring regulatory and organizational compliance.
