Job summary

CVS Health is seeking an Analyst, Care Manager in New York to deliver comprehensive care management for MLTC members. You will conduct psychosocial assessments, develop person-centered plans, and coordinate long‑term services with medical and community resources.

The role emphasizes collaboration with members, caregivers, and interdisciplinary teams to improve health outcomes and access to services. 40 hours weekly with a full benefits package.

Qualifications

• Active and unrestricted New York State LMSW or LCSW licensure as required.
• Minimum 2 years of experience in care management or related healthcare settings.
• Knowledge of NY Medicaid, Medicare, MLTC, LTSS.
• Experience conducting psychosocial assessments and person-centered care planning.
• Strong communication, care coordination and problem-solving skills.
• Ability to travel within the service area for member visits.
