Job summary

Intermountain Health is seeking a Remote Care Management Specialist to support identified high-risk patients from our Nevada Central Office. The role requires collaboration with the care team, patients, families, and providers to ensure effective transition management and coordinated care.

You will conduct pre-screening, referral screening, patient scheduling, and follow-up communications, while adhering to HIPAA and Intermountain policies.

Qualifications

• Three years of experience in patient care, care management, transition/discharge planning, medical assistance, healthcare coaching, or patient care coordination - and - Certification, Associate Degree, or Bachelor's degree in a healthcare field.
• Excellent interpersonal and communication skills.
• Ability to adapt quickly as needs arise.
• Knowledge of available health resources.

Responsibilities

• Screen, monitors pending and prioritized referrals. Maintains a record of the average daily caseload and workload.
• Promptly contacts new patients by phone to introduce the program, obtain verbal consent to participate, and schedules the initial assessment/evaluation.
• Performs patient follow-up calls to reinforce education, self-management, and care planning actions with the care team.
• Lead interdisciplinary care conferences and document in the medical record per guidelines.
