Job summary

Alliance Health is seeking a Practice Transformation Specialist to improve patient outcomes by enhancing care management, CQI, and data-informed decision making across AMHs and CMAs. The role emphasizes training, coaching, and data reporting to drive value-based care shifts.

The position supports providers remotely with periodic in-person meetings at Alliance offices. Ideal candidate brings clinical/health services background and strong analytics to guide 1915(i) and HR.1 initiatives.

Qualifications

• Bachelor’s degree in nursing, social work, or other human services related field and five years of post-degree healthcare or managed care experience.
• Experience leading healthcare quality and process improvement initiatives in an operational setting is preferred.
• Experience in public speaking, mediation, and training is preferred.

Responsibilities

• Provide support to network providers by improving workflows, billing readiness, and data-driven care management.
• Educate practice staff and CINs on CQI principles, interventions, and data-driven improvements.
• Present performance data in an accessible format to drive provider improvement.
• Collaborate with AMHs, CMAs, CINs to integrate admin, financial, and clinical data for better outcomes.
• Facilitate adoption of 1915(i) processes and HR.1 impacts for members.
• Deliver practice-level education, toolkits, and resources for improving member care.
