Job summary

CommonSpirit Health is seeking a Care Transformation RN for a remote role with travel to support enterprise-wide Virtual Health Services. The position emphasizes delivering comprehensive, coordinated health services remotely, using telehealth and digital tools to assess and manage patient care across disciplines.

The role requires a BSN with 5 years of acute care nursing, 3 years of preceptorship, and 2 years in leadership, with CPR certification.

Qualifications

Bachelor's of Nursing and 5 years of overall acute care nursing experience.
3 years of nursing preceptor experience.
2 years in a nursing leadership role.
Basic Life Support - CPR.
Virtual care and critical care experience is preferred.

Responsibilities

Serve as a Virtual RN (VIC RN) for 50% of the role, providing direct patient care during implementations including travel.
Virtually collaborate with interdisciplinary teams, assess patient status, and develop comprehensive care plans.
Respond to patient issues, educate patients/families, and advocate for their needs while using technology effectively.
Coach and mentor bedside nurses, focusing on quality, cost containment, and compliance with standards.
Dedicate 50% to Care Transformation: orienting/mentoring VIC RNs, leading simulations and education, and monitoring workflows/technology.
Act as a program leader, providing leadership support, troubleshooting barriers, and guiding staff adjustment based on census.
