Net Health co is looking for a Remote Care Navigator to support patients in their recovery through effective remote monitoring and communication. As part of the team, you'll provide assistance with home-exercise therapy and help users thrive through our technology platform. Candidates should be empathetic, tech-savvy, and possess strong communication skills to ensure patient success.

Experienced candidates with an accredited physical therapy background and an active PTA license will be given priority. This full-time role offers a competitive hourly wage between $20.80 and $26.00, plus potential performance bonuses.

Qualifications

• Minimum of 2 years experience in musculoskeletal health preferred.
• Experience in behavioral modification; Health Coach Certification is a plus.
• Ability to multi-task and manage different types of users.

Responsibilities

• Provide remote monitoring services for patients' home-exercise therapy.
• Assist users in therapeutic exercise and functional training.
• Maintain records and provide feedback to clinicians.
