Job summary

Qualifications

• Bachelor’s degree required; 2–4 years related experience.
• Graduate from Accredited School of Nursing if holding clinical licensure.
• Current state nursing license preferred.

Responsibilities

• Evaluates member needs and barriers to care; recommends and coordinates the care plan.
• Develops or contributes to personalized care/service plans and identifies providers or community resources.
• Provides psychosocial and resource support to members and caregivers to access local services.
• Coordinates between member/family and care provider team to ensure timely access to services.
• Monitors progress toward care plan goals and collaborates with providers for revisions as needed.
• Collects and maintains member information to ensure compliance with regulations.
