Job summary

Qualifications

  • At least 2 years of health care experience including LTSS and care management.
  • Valid driver's license, reliable transportation for travel.
  • Knowledge of community resources.
  • Ability to work independently with minimal supervision.
  • Strong verbal and written communication skills.
  • Proficiency with Microsoft Office and online portals.
  • Experience with individuals with substance use disorders (where required).

Responsibilities

  • Complete comprehensive member assessments within regulated timelines, including in-person home visits.
  • Facilitate waiver enrollment and disenrollment processes.
  • Develop and implement care plans with members and care networks.
  • Monitor care plans, document interventions, and adjust as needed.
  • Promote integration of LTSS with behavioral health and community resources.
  • Assess medical necessity and authorize waiver services.
  • Collaborate with the interdisciplinary care team for approvals.
  • Educate and motivate change using motivational interviewing.
  • Identify barriers and provide coordination for psycho/social, financial, and medical obstacles.
  • Identify critical incidents and develop prevention plans; travel 25-40% may be required.
