Position: Care Manager I
Work Arrangement: Fully Remote
Schedule: Monday–Friday, 8: 00 AM–5: 00 PM CST
Contract Duration: 6+ months, with potential for extension and/or conversion to a full-time position
Job Description:
We are seeking an experienced, licensed Social Work or Behavioral Health professional to support member engagement and care coordination in a fully remote environment.
Key Responsibilities
- Conduct outreach to members, educate them about needed healthcare services, help address barriers to care, and coordinate appointments and appropriate resources.
- Conduct outbound calls to members to educate them about needed healthcare services and support engagement in care.
- Identify barriers to care and assist members in accessing appropriate services and resources.
- Schedule appointments with provider offices as needed.
- Connect members with appropriate community, healthcare, and support resources.
- Accurately document all outreach attempts, member interactions, outcomes, and follow-up activities in the case management platform.
- Maintain confidentiality and comply with HIPAA requirements and organizational policies.
- Collaborate with care management, provider, and quality teams to improve member engagement and help close gaps in care.
- Complete follow-up activities and maintain accurate and timely case documentation.
Required Skills:
- Minimum 5 years of social work experience.
- Must hold an active professional license/certification permitting practice in Nebraska.
- Strong communication, interpersonal, organizational, and documentation skills.
- Ability to work independently in a fully remote environment.
- Ability to handle protected health information in accordance with HIPAA and applicable organizational policies.
Licenses or Certifications:
- LCSW / LISW or equivalent Clinical Social Work license
- LPC / LPCC / LMHC or equivalent Professional Counselor/Mental Health Counselor license
- LMFT –Licensed Marriage and Family Therapist
