Job summary

CenterLight Healthcare System is seeking a Care Manager responsible for overall care management and quality of care for participants, coordinating holistic care plans, and ensuring services meet member needs. You will work within an IDT, coordinate services such as ADLs/IADLs, and monitor care plans for effectiveness; requires NY license and nursing/home health leadership experience.

Remote position with potential for some on-site work; salary range $88,000–$98,000 per year, with bilingual

Qualifications

• Education requirements include a bachelor’s degree in nutrition or related field; a Master’s in Nutrition is preferred.
• Experience includes minimum two years in a management role in home health or related settings; customer service experience required.
• License/authorization to practice in New York State is required.

Responsibilities

• Participates in care planning meetings and coordinates holistic care plans.
• Reviews discipline-specific documentation for quality and addresses deficiencies with staff.
• Coordinates services with the IDT to meet member health needs and goals.
• Documents services per CLHC standards and regulations.
• Ensures care transitions across sites and collaborates with care team.
