Qualifikationen

• Education qualifications per 10A NCAC 27 .0104 for QP eligibility.
• A MH/SU license (including associate-level) or NC Substance Abuse Board certification.
• An RN with four years of full-time MH/SU/IDD experience.
• Master's degree in a human service field plus at least one year of MH/SU/IDD experience.
• Bachelor's degree in a human service field plus at least two years of MH/SU/IDD experience.
• Bachelor's degree in a non-human service field plus at least four years of MH/SU/IDD experience.

Aufgaben

• Care Management Service Delivery - fosters and facilitates the care management process; conducts comprehensive assessments and updates.
• Implements Tailored Care Management Planning with the person supported and other team members.
• Monitors ongoing care plan implementation and updates every 12 months or on significant changes.
• Maintains a positive and professional image and follows policies to ensure accountability.
• Cross-functional collaboration with Integrated Care Team, community organizations, and RHA departments.
• Promotes person-centered care focusing on strengths, needs, and safety.
• Uses data strategically to support integrated care and ensure data sharing per policies.
• Maintains a safe and healthy environment and ensures reimbursement documentation.
