Qualifications

• 2+ years in behavioral health, social services, or human services.
• 2+ years using PCs and MS Office (Outlook, Word, Excel, SharePoint).
• 2+ years in children's mental health, child welfare, or related fields.
• 2+ years in family systems, case management, or child development.
• 2+ years experience with Ohio delivery systems and local networks.
• Willing to travel 50%+; may travel to New Albany office for trainings.
• Reliable transportation; mileage reimbursed.
• Willing to work beyond core hours as needed.

Responsibilities

• Assess referred members' needs/eligibility and recommend a care plan.
• Coordinate and monitor care plan activities and progress.
• Collaborate with case managers and health teams to improve outcomes.
• Identify and escalate quality of care issues through established channels.
• Provide coaching and support to empower informed health choices.
• Act as single point of contact for members and address gaps in care.
