Job summary

Qualifications

• Current Florida LPN license or CMA/RMA certification preferred.
• Experience in care management, population health, or transitions of care.
• Proficient with electronic health records (EHR) and care coordination tools.

Responsibilities

• Assist the RN Care Manager with individualized care plans and monthly chart reviews.
• Perform outreach to patients and caregivers to identify barriers to care.
• Support transitions of care after visits or hospital stays and coordinate follow‑up.
• Educate patients on chronic disease management and preventive care.
• Maintain accurate documentation in the EHR and track outcomes.
• Coordinate services with providers, hospitals, home health, and community resources.
