Job summary

TimeDoc Health, Inc. is seeking a part-time Care Coordinator for remote work to support Chronic Care Management (CCM) with high-volume phone calls. You will coordinate care, educate patients on care plans, and document interactions in the EMR, maintaining HIPAA compliance.

The role emphasizes structured schedules, daily productivity metrics, and independence in a fully remote environment, ideal for clinicians with CMA/RMA or LPN backgrounds. Flexible hours and home-office setup are required.

Qualifications

• 1–3 years of CMA/RMA or LPN experience
• Active certification or multistate license for LPNs
• Strong verbal and written English communication
• Comfortable spending most of the day on the phone
• Experience with EMR/EHR systems
• Able to work independently in a remote setting

Responsibilities

• Conduct monthly CCM phone calls with assigned patients/practice
• Complete required patient interactions to meet CCM goals
• Educate patients on chronic conditions and care plans
• Coordinate care with provider offices, pharmacies and community resources
• Identify gaps in care and social determinants of health
• Create/update personalized care plans in the EMR
• Document all patient interactions accurately
• Maintain HIPAA compliance and patient confidentiality
