Job summary

Lancesoft is seeking a Healthcare Consultant I / Care Coordinator in Florida. The role involves telephonic and field case management, with up to 75% travel and 25% remote work. Candidates must reside in Miami Dade County and have a bachelor’s degree.

Qualifications

• Case management experience required.
• Long term care experience preferred.
• Microsoft Office including Excel competent.
• Bilingual Spanish/English REQUIRED.

Responsibilities

• Telephonic and/or face-to-face assessments, planning, implementing, and coordinating all case management activities with members to evaluate medical needs and facilitate wellbeing.
• Manage a caseload including supportive and medically complex members, developing proactive plans to improve short- and long-term outcomes.
• Coordinate care plan activities, monitor progress, and liaise with PCP and skilled providers to ensure appropriate services.
