Job summary

A-Line Staffing Solutions is seeking a Hybrid LTSS Social Work / Human Services Case Manager to support Medicaid Long-Term Care enrollees. The role blends remote documentation with field visits in Miami-Dade County, with up to 75% travel.

Candidates must reside in Homestead/Florida City (33033, 33032), hold a Bachelor's in Social Work or related field, and be bilingual in Spanish and English. Work schedule is Monday–Friday with paid travel time and mileage reimbursement.

Qualifications

• Case management experience required.
• Bachelor's degree in Social Work or related Human Services.
• Must reside in Homestead/Florida City (33033, 33032) and be U.S. authorized to work.

Responsibilities

• Coordinate case management services for Medicaid Long-Term Care members.
• Conduct comprehensive member assessments using care management tools and data review.
• Develop, implement, and monitor individualized care plans.
• Collaborate with interdisciplinary care teams to achieve optimal health outcomes.
• Track progress and update documentation per program requirements.
