Formación

• 3–5 years' experience in Social Work, Nursing or Healthcare-related field.
• Experience in utilization management, quality assurance, home or facility care, community health or long-term care.
• Cost/benefit analysis, negotiation and cost containment are desirable.
• Knowledge of referral coordination to community resources.
• Ability to analyze and interpret data for decision making.
• Excellent verbal and written communication with clinicians and service providers.

Responsabilidades

• Coordinate care of clients using assessment, care planning, implementations and monitoring.
• Promote appropriate use of clinical and financial resources to improve outcomes.
• Mentor new team members as appropriate.
• Provide care coordination for behavioral health cases requiring intensive interventions.
• Develop and monitor care plans with social, physical, mental and community resources.
• Advocate for members and address gaps in care.
• Educate providers and staff on care coordination best practices.
