  • Deliver care to medically complex patients across a variety of clinical settings and care transitions.
  • Provide cardiovascular consultation and clinical guidance for high-risk and medically complex patients.
  • Serve as a clinical resource to interdisciplinary care teams to support evidence-based cardiovascular care and treatment planning.
  • Partner with multidisciplinary teams to develop and implement care strategies that improve quality, reduce utilization, and enhance patient outcomes.

  • Experience caring for high-risk, medically complex, and/or Medicare Advantage patient populations preferred.
  • Experience with telehealth, value‑based care, population health management, and interdisciplinary team‑based care preferred.
