The Cardiologist will collaborate with internal medicine physicians, nurse practitioners, and specialists in cardiology, endocrinology, pulmonology, and behavioral health to support patients managing multiple chronic conditions such as heart failure, diabetes, COPD, hypertension, chronic kidney disease, and other cardiovascular conditions.

The ideal candidate brings strong clinical judgment, experience managing medically complex patient populations, and a commitment to improving quality outcomes through proactive, evidence‑based cardiovascular care. This role supports care planning, clinician education, and interdisciplinary collaboration to help reduce avoidable hospitalizations, improve patient outcomes, and advance health equity.

Responsibilities
  • Deliver cardiovascular care to patients with multiple chronic conditions via telemedicine
  • Apply clinical guidelines to determine appropriate interventions and care pathways
  • Oversee quality metrics for patient panels, including blood pressure control and medication adherence
  • Reduce avoidable hospitalizations and emergency visits through targeted care strategies
  • Supervise Nurse Practitioners and support their clinical development
  • Collaborate with the Chief Cardiologist to deliver educational training to Nurse Practitioners and other clinical team members.
  • Collaborate with Market Physician Executives on care plans for high-risk or frequently hospitalized patients
  • Participate in clinical committees and peer‑to‑peer consultations
  • Demonstrate clinical expertise across a broad range of cardiovascular and medically complex patient conditions.
  • Use data tools to monitor, report, and enhance care quality and utilization
  • Deliver care to medically complex patients across a variety of clinical settings and care transitions.
  • Provide cardiovascular consultation and clinical guidance for high‑risk and medically complex patients.
  • Serve as a clinical resource to interdisciplinary care teams to support evidence‑based cardiovascular care and treatment planning.
  • Partner with multidisciplinary teams to develop and implement care strategies that improve quality, reduce utilization, and enhance patient outcomes.
Position Requirements
  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree
  • Graduate of an Accredited Cardiology Training Program
  • Board Certification in Internal Medicine
  • Board Certified in Cardiology * Required Licensure
  • Current state medical license without restrictions to practice and free of sanctions from Medicaid or Medicare. Willingness to become licensed in multiple states as company and member needs require.
  • Experience educating clinicians on cardiac conditions
  • Experience caring for high‑risk, medically complex, and/or Medicare Advantage patient populations preferred.
  • Experience with telehealth, value‑based care, population health management, and interdisciplinary team‑based care preferred.
  • Demonstrated communication and collaboration skills with the ability to educate and influence clinicians across multiple specialties.
  • Ability to use data and quality metrics to drive improved cardiovascular outcomes and patient care.
