Job summary

Monogram Health is seeking a full-time Lead Palliative Care Physician to join our in-market team in Texas. The role focuses on in-home care for seriously ill patients, including CKD/ESRD, COPD, and CHF, working with the Market Physician Executive.

You will drive KPIs, collaborate with care teams, and support transitions to hospice when appropriate, with a base salary of $260k–$340k plus bonus potential and comprehensive benefits.

Qualifications

  • Must hold a state medical license without restrictions.
  • Board certification in Hospice and Palliative Medicine.
  • Willingness to obtain hospital privileges and multi-state licensing.
  • Experience with CKD/ESRD, COPD, CHF in population health context.

Responsibilities

  • Collaborate with MPE and care team to identify patients for palliative care.
  • Develop and present metrics to executive leadership.
  • Drive KPIs for patient panel through specified interventions.
  • Treat panel of patients in-home to improve outcomes.
  • Manage symptoms impacting quality of life (pain, depression, etc.).
  • Refer to Monogram-based or local specialists for care.
  • Conduct goals of care and family meetings; document wishes.
  • Support patients with IDT including Palliative Care Chaplain.
  • Assist with hospice transitions as appropriate.
  • Build relationships with local providers to improve market alignment.
  • Participate in monthly KPI meetings with RMD/CMO.
  • Educate and support team on palliative and hospice care.

Position: Lead Palliative Care Physician

Monogram Health is seeking a full-time physician specializing in palliative care to join our team amid continued nationwide growth. This is an in‑market, field‑based position requiring residence within one of our approved territories. As an in‑market specialist, this physician will care for seriously ill patients in their community, including individuals with CKD/ESRD, COPD, and CHF. The Lead Palliative Care Physician will work closely with the Market Physician Executive (MPE) and primarily deliver care in the home.

Compensation

The base salary range for this role is $260,000 to $340,000 and reflects the range of experience and qualifications we may consider. Final compensation will be determined based on geographic location and job-related factors such as experience, skills, education, certifications, and internal compensation considerations. While a larger range is posted, most offers will typically fall within a narrower band based on these factors.

In addition to base salary, this role is eligible for additional compensation, including bonus opportunities and a comprehensive benefits package.

Roles & Responsibilities
  • Collaborate with MPE and the care team to proactively identify patients who qualify for palliative care services and build a patient panel
  • Developing and presenting metrics to executive leadership, as requested
  • Responsible for the outcomes for patient panel, driving both predetermined clinical and financial Key Performance Indicators (KPIs) through interventions outlined below
  • Treat panel of patients in-home to drive healthcare outcomes
  • Evaluate and treat symptoms that may interfere with quality of life, like pain, pruritis, depression, anxiety, insomnia, etc.
  • Refer members to appropriate specialists to drive clinical care (Monogram-based specialists or locally-aligned specialists)
  • Conduct goals of care and family meetings as needed and document patient/family wishes in state AD/POLST forms
  • Support patients utilizing our IDT, including Palliative Care Chaplain
  • Assist with hospice transitions as appropriate
  • Develop key relationships with local providers (PCP, nephrologists, specialists) to improve market alignment
  • Participate in monthly meetings with RMD/CMO in areas of KPI and expectations around KPIs
  • Educate and support team around palliative care, hospice care, advance directives, symptom assessment, and communication
Position Requirements
  • Must be willing and able to obtain hospital privileges at required facilities.
  • Desire to practice in a forward-looking renal care environment focused on population health management
  • Demonstrated experience applying evidence-based clinical criteria
  • Comfort and skill in goals of care discussions and advance care planning
  • Experience with high-need Medicare Advantage and managed Medicaid populations, as well as commercial populations
  • Experience integrating acute and behavioral health care and appreciation of social determinants of health and the value of social workers
  • Expertise in managing medication panels for complex patients
  • Current state medical license without restrictions to practice and free of sanctions from Medicaid or Medicare.
  • Willingness to become licensed in multiple states as company and patient needs require
  • Doctorate Degree in Medicine
  • Board Certified in Hospice and Palliative Medicine
  • Must have reliable transportation
