Qualifications

• 1-3 years of nursing experience required.
• 3+ years as Nurse Navigator or patient educator preferred.
• Associate degree in nursing required; BSN preferred.
• RN licensed in the state of practice; Basic Life Support required if remote.
• Eligible to practice in state of Texas (Dallas/Fort Worth market).

Responsibilities

• Serve as patient advocate and educator for patients with varied medical conditions.
• Assist with navigation through complex care pathways and healthcare systems.
• Coordinate appointments, education, and access to community resources.
• Identify barriers to care and connect patients with resolutions.
• Participate in multidisciplinary meetings and share patient data for program tracking.
