Job summary

HCA Healthcare in the Dallas/Fort Worth area is seeking a Nurse Navigator to join a multidisciplinary team. This remote role supports patients with diverse medical conditions, guiding access to care and coordinating the plan of care with physicians and facilities.

Required: 1–3 years of nursing experience and an Associate’s Degree in Nursing with RN licensure; BLS preferred. The Navigator educates patients and families, facilitates appointments, and connects with community resources to improve

Qualifications

• 1–3 years of nursing experience required.
• 3+ years of Nurse Navigator and/or patient educator in service line preferred.
• Current RN license required; BLS preferred for remote position.

Responsibilities

• Serves as a patient advocate.
• Assesses patient medical and psychosocial needs related to navigation.
• Identifies barriers to care and facilitates access to resolutions.
• Educates patients and families, empowering them to manage healthcare needs.
• Guides patients through the care pathway and coordinates appointments.
• Coordinates with physicians to ensure adherence to treatment plans.
• Supports data collection to track navigation program performance.
