Job summary

HCA Healthcare is seeking a Nurse Navigator to join a multidisciplinary team in a remote role serving the Dallas/Fort Worth, TX area. The position emphasizes advocacy, education, and coordination of care for patients with cardiovascular and other conditions.

The role requires RN licensure, 1–3 years nursing experience, and 3+ years in navigator/education roles; BSN preferred. Work from home with residency in the Dallas/Fort Worth market.

Qualifications

• 1-3 years of Nursing experience required.
• 3+ years as Nurse Navigator and/or patient educator in service line preferred.
• Associate Degree in Nursing required; BSN preferred.
• RN licensure in the state of practice required.

Responsibilities

• Advocate for patients and coordinate navigation across the care continuum.
• Assess patients' medical, psychosocial, and access needs to facilitate timely care.
• Educate patients and families and connect with community resources.
• Guide patients through appointments, transitions, and care plans with providers.
• Attend multidisciplinary meetings and document patient data to support program goals.
