Job summary

MountainStar Healthcare is seeking a Nurse Navigator to support patients through complex care, coordinating plans and guiding access to care to improve outcomes. You will need 1–3 years of nursing experience, an Associate Degree in Nursing (Bachelor’s preferred), and an active RN license with multi-state eligibility.

Responsibilities

• Serves as a patient advocate and educator for patients with various medical conditions.
• Coordinate the defined plan of care and assist with appointments and access to resources.
• Educate patients and families, promote adherence to treatment, and support care transitions.
• Attend Multidisciplinary Meetings and participate in care planning conferences as required.
• Gather and document patient data to monitor navigation program performance.
