Carda Health seeks a Patient Retention CEP to collaborate with clinicians and founders on digital care delivery and retention strategies. You will proactively contact patients at risk of exiting the program to address concerns, illustrating long-term benefits and driving ongoing participation in telehealth rehab.

Ideal candidates bring cardiopulmonary rehab experience, strong interpersonal skills, and comfort with mobile tech.

Formación

• Minimum 1+ year of relevant clinical experience in cardiopulmonary rehab.
• Strong interpersonal and patient relationship skills.
• Proficiency with mobile technology and wearables/tablets.
• Excel proficiency for data tracking and reporting.
• Bachelor’s in Exercise Physiology; Master’s preferred; ACSM-CEP preferred.

Responsabilidades

• Collaborate with clinical team to improve digital care delivery and retention strategies.
• Reach out proactively to at-risk patients to address concerns and sustain participation.
• Demonstrate long-term benefits of the program and foster patient relationships.
• Support smooth patient and client experience with the technology team.
• Maintain repeatable processes and ensure high-quality rehab delivery.
