Job summary

CVAUSA is seeking a Chronic Care Management-Health Coach for a remote day shift. You will build patient coaching relationships, support care plans and quality of life, and liaise with the cardiology care team to improve outcomes.

Responsibilities include communicating findings, monitoring adherence, and assisting patient enrollment while following clinical protocols. This remote role offers full-time hours and a patient-centered CCM platform experience.

Qualifications

• High School diploma or GED.
• 1 year of remote chronic care management experience.
• RN/MA/CNA certification required.
• Experience with EMRs – Athena, NexGen, GEMMS and e-clinical works.

Responsibilities

• Develop a patient coaching relationship supporting the care plan.
• Act as empathic support for patients and families with chronic conditions.
• Communicate abnormal findings to the cardiology care team.
• Monitor adherence to care plans and encourage participation.
• Assist with enrollment of new patients and liaise with the practice.
