Position: Behavioral Health Specialist

Candidates must currently be licensed as an LCSW in either AZ or TX to qualify

Monogram is seeking a Behavioral Health Specialist to complete assessments and provide evidence-based psychotherapy services to its patients. The Behavioral Health Clinician will assess patient needs, make referrals to appropriate behavioral health resources and specialists, and provide short-term counseling and treatments as indicated. The clinician will employ various strategies, approaches, and techniques to manage patients' physical, environmental, and psychosocial health issues.

Roles and Responsibilities
  • Conduct assessments and screening to identify behavioral health conditions early.
  • Provide accurate clinical assessments of mental and behavioral health conditions.
  • Support care team with treatment goals and plans.
  • Detect at-risk patients and develop plans to prevent worsening of complex medical conditions.
  • Manage psychosocial aspects of diseases and make referrals as appropriate.
  • Address lifestyle and health risk concerns with thoughtful interventions.
  • Provide brief, evidence-based interventions.
  • Give timely feedback to medical providers about care and treatment.
  • Advise care team on virtual vs. in-person care needs and referrals to mental health programs.
  • Help patients understand and change behaviors, develop healthier coping mechanisms.
  • Maintain detailed notes on each patient.
  • Collaborate with families for feedback on treatment and progress.
  • Assess patient improvement over time.
  • Participate in multidisciplinary team meetings and contribute to treatment plans.
Position Requirements
  • Must have an active LCSW and be able to practice independently (additional state licenses may be required).
  • Excellent knowledge of behavioral medicine and evidence-based treatments.
  • Deep understanding of behavior therapy models and techniques (e.g., Motivational Interviewing, CBT).
  • Strong problem-solving skills.

  • Role involves telehealth and occasional in-home visits across the state.
