Role Description:

The purpose of a Behavioral Health Specialist at Oak Street Health is to provide care management services and evidence-based short-term psychotherapy to patients with behavioral health needs. They are a key resource for Care Teams across Oak Street Health primary clinics, responsible for coordinating behavioral healthcare in collaboration with Care Teams and a psychiatric consultant. The role includes assessing patient needs, making referrals, and providing short-term counseling, with potential for remote work based on performance.

Core Responsibilities:
  • Collaborate with primary care teams to identify patients for the Behavioral Health Program
  • Screen and assess patients for mental health and substance use disorders
  • Develop care plans and facilitate referrals to psychiatric or external resources
  • Provide short-term counseling and evidence-based treatments
  • Coordinate care with psychiatric consultants, including telehealth handoffs
  • Maintain accurate documentation in electronic medical records
  • Participate in regional and organizational initiatives to improve the program
  • Cover multiple clinics as needed
  • Perform other duties as assigned
Qualifications:

Required:

  • Master’s Degree in mental health counseling or related field from an accredited school
  • Highest clinical level license or equivalent
  • Experience in screening and assessment of mental health/substance use disorders
  • Knowledge of brief, structured intervention techniques (e.g., Motivational Interviewing, CBT)
  • US work authorization
  • Embodies Oaky values

  • Proficiency in structured intervention techniques
  • Knowledge of differential diagnoses

Work Details:

40 hours per week, full-time. Salary range: $66,575 - $142,576, depending on experience and location. Eligible for bonuses and incentives.
